Pricing
All prices include shipping. All items are shipped within 4-6 business days by U.S. Postal Service mail. Pricing for members includes MDA members only. Your ADA number will be verified.
Brochure Return/Refund
All sales final after 10 business days from date materials shipped from MDA. Unopened merchandise may be returned within 10 days of order shipment date. Only unused and unopened brochures in original packaging will be accepted. All returns are subject to a minimum 15% restocking fee. Returned shipping is the responsibility of the sender. Please contact the MDA to arrange for a refund before shipping materials for return.
